We offer telehealth visits statewide in Florida. For in-home visits, we are currently serving communities in St. Johns County and Duval County between Ponte Vedra and St. Augustine and communities South of 295 and East of the St. Johns River, as well as Nocatee and the Beaches communities, including Jacksonville Beach up to Jacksonville Golf and Country Club and Atlantic Beach.
