**We do not accept or process insurance claims at this time. We will provide a receipt of service and a superbill, which can be submitted to your insurance for an out-of-network claim. We fall under Telehealth and Urgent Care, which many insurance plans cover. We accept self-pay, debit/credit, HSA and FSA. If you need assistance processing a claim, we recommend using www.reimbursify.com.

Membership Options
We are currently serving communities in St. Johns County between Ponte Vedra and St. Augustine Beach and Duval County/St. Johns communities South of 295 and East of the St. Johns River, as well as the Beaches communities. If outside our service area radius, a travel fee will be determined on a case by case basis.
